Renal MDT & flow Coordinator

Overview

Secondment opportunity for a band 4 Regional Renal flow coordinator based at the Royal Liverpool Hospital. The successful candidate will work with the MDT to coordinate patient flow across regional units and acute hubs, including HD coordination for inpatients. This role will support the Renal Assessment Unit.

Responsibilities

• Coordinate patient flow across regional units and acute hubs in collaboration with the multidisciplinary team.
• HD coordination for inpatients.
• Support the Renal Assessment Unit and Directorate staff in coordinating, tracking and organising multi-disciplinary team meetings for patients.
• Collect data relating to the patient journey; work to deadlines and ensure data quality.
• Collaborate with clinical information staff, clinical teams across the Trust, and Support Services (e.g., Radiology and Pathology).

Qualifications/Experience

• Recent Renal Experience at Band 3 is Essential.
