Working across areas including Islington, Finchley, Hampstead, Muswell Hill, and Enfield, you will provide clinical care, patient education, and ongoing support while working closely with multidisciplinary healthcare teams.

Job responsibilities
This is an exciting opportunity for a Registered Nurse looking for greater autonomy, continuity of care, and the chance to build meaningful long-term relationships with patients in the community.
You will work closely with multidisciplinary healthcare teams including dietitians, pharmacists, hospital teams, and other healthcare professionals to deliver safe, high-quality, patient-centred care.
Main Duties and Responsibilities
- Deliver high-quality nursing care in line with service operating procedures and clinical protocols
- Support patients receiving IV therapies and TPN within community and homecare settings
- Conduct home visits for therapy initiation, assessment, follow-up care, and ongoing patient support
- Provide education and training to patients, carers, and families to promote safe self-management and treatment adherence
- Build strong therapeutic relationships with patients while delivering compassionate and personalised care
- Maintain accurate clinical documentation and electronic patient records
- Monitor patient progress and escalate adverse events or clinical concerns appropriately
- Maintain and manage clinical equipment and materials safely and effectively
- Work collaboratively with multidisciplinary healthcare teams to ensure positive patient outcomes
- Participate in team meetings, service improvement initiatives, and ongoing professional development activities
- Adhere to safeguarding, infection prevention, governance, and regulatory requirements at all times

Qualifications
- Registered Nurse with current NMC registration (Adult or Childrens Nurse)
- Minimum of 1 year post-registration experience
- Full UK or Irish driving licence

- Experience in the management and care of vascular access devices, including PICC lines, central venous access devices (CVADs), or similar intravenous access devices
