You’ll be working in a dynamic hospital setting supporting timely and safe discharges. The service operates across a 200-bed pathway, where individuals are supported to return home with packages of care, with the option for re-referral if ongoing support is required.

Your role will involve completing assessments within 14–28 days, undertaking 6–12 week reviews, and contributing to a light-touch triage system that ensures people receive the right level of support at the right time. You’ll play a key role in ensuring smooth transitions from hospital to home, promoting independence and reducing readmissions.

You’ll be joining a collaborative and supportive multidisciplinary team, working closely with healthcare professionals to deliver person-centred discharge planning and ongoing care.
