Job Introduction

This role provides an exciting opportunity to work at the heart of integrated pathways, supporting people to leave hospital safely and with the right support in place. It is a fast‑paced environment with a strong focus on delivering safe and timely hospital discharges through short‑term, outcome‑focused interventions. You will play a key role in delivering strengths‑based practice, completing Care Act assessments that promote independence, wellbeing and positive outcomes.
The position is predominantly hospital‑based. Most of the time will be spent working on‑site as part of a multi‑disciplinary team. Occasional home visits may be required but the role is primarily based within an acute hospital setting, enabling responsive and collaborative discharge planning.
Responsibilities
- Undertake strengths‑based Care Act assessments and reviews.
- Support safe, timely, and person‑centred discharges.
- Deliver short‑term interventions to facilitate discharge planning.
- Work within integrated pathways alongside health and social care professionals.
- Develop and maintain close working relationships with health colleagues and partner agencies.
- Contribute to safeguarding and risk management processes.
Locations
Royal Liverpool Hospital, Aintree University Hospital, Broadgreen Hospital. Accommodate individual preferences regarding base location; flexibility to work across sites may be required in line with service needs.
