NHS Highland’s Learning Disability Service is seeking a compassionate, motivated, and experienced Senior Outreach and Day Support Worker to join our dedicated team. You will play a key role in supporting adults with learning disabilities and other vulnerabilities to live fulfilling, independent lives within their own homes and communities.
Our service supports individuals in both shared and individual tenancies, providing 24‑hour, in‑reach and outreach provision. You will also work closely with day services and collaborate with the wider integrated health and social care team. Key Responsibilities
Care & Support
- Act as key worker for a number of tenants, developing, reviewing and delivering personalised support plans.
- Provide high‑quality, person‑centred care to individuals with varying support needs in their own homes and community settings.
- Support individuals with personal care, medication administration, daily living skills, social and leisure activities, and community participation.
- Encourage and enable individuals to gain confidence, maintain independence, and pursue meaningful activities including employment or volunteering.
Leadership
- Act as shift leader, overseeing service delivery and staff allocation.
- Guide, support, and mentor Support Workers to deliver safe, effective, outcome‑based care.
- Participate in risk assessments, service improvement, incident reporting (including DATIX), and care plan evaluations.
- Take part in the service on‑call rota.
Professional Standards & Administration
- Maintain accurate care records, reports, and documentation.
- Ensure compliance with NHS Highland policies, SSSC codes, infection control, health and safety standards, and medication procedures.
- Work collaboratively with the multidisciplinary team, families, carers, and external agencies.
- Uphold confidentiality and professional boundaries at all times.
What We’re Looking For
You should bring
- Minimum 3 years’ experience in a care or support setting.
- SVQ Level 3 (or above) in Health & Social Care and compliance with SSSC registration requirements.
- Strong understanding of the needs of vulnerable adults, including those with learning disabilities, physical disabilities, dementia, and complex needs.
- Experience in contributing to or modifying support plans.
- Training and competency in administering medication (or willingness to achieve).
- Excellent communication—written, verbal, and interpersonal.
- Ability to work independently and as part of a team.
- Good organisational and time‑management skills.
- A full driving licence.
