Responsibilities

• Engage with people to discuss what matters to them and their carers.
• Use local information and advice to support individuals in finding practical solutions.
• Collaborate with social care, health, probation and housing teams to deliver integrated support.

Qualifications

• Registered with Social Work England.
• Holder of a valid UK driving licence.
• Appointment subject to an enhanced DBS check.
