Sheffield Health Partnership University NHS Foundation Trust is seeking a dedicated and compassionate Adult Home Treatment Social Worker to join its expanding Home Treatment Team (HTT) based at the Crisis Hub, Longley Centre in Sheffield. This is an exciting opportunity for an experienced Mental Health Social Worker to become part of a forward-thinking crisis service focused on supporting individuals experiencing acute mental health challenges within the least restrictive environment possible.

The Home Treatment Team provides rapid assessment, intervention, and ongoing support for service users experiencing mental health crises as an alternative to hospital admission. Working within a highly collaborative multi-disciplinary team, you will play a critical role in crisis intervention, care planning, safeguarding, and recovery-focused support while helping individuals remain safe and supported within their communities.

  • Conduct comprehensive mental health crisis assessments for new referrals.
  • Lead crisis intervention planning and coordinate onward referrals to appropriate services.
  • Deliver evidence-based acute crisis interventions within community settings.
  • Develop and review person-centred care plans tailored to individual needs.
  • Assess and manage clinical and safeguarding risks effectively.
  • Coordinate shift activities, including triaging referrals and prioritising workloads.
  • Work collaboratively with Community Mental Health Teams, inpatient services, and partner agencies.
  • Support service users with housing applications, social care pathways, and benefits-related matters where appropriate.
  • Maintain accurate and timely electronic clinical records in accordance with Trust policies.
  • Provide supervision, mentorship, and support to junior staff and learners.
  • Participate in multidisciplinary meetings, safeguarding enquiries, and Mental Health Act-related processes.
  • Contribute to service development initiatives, audits, and continuous improvement activities.

Candidate Requirements

Required

  • Registered Social Worker with current Social Work England registration.
  • Evidence of continued professional development.
  • Experience working as a Band 5 or Band 6 Mental Health Social Worker within community or multidisciplinary settings.
  • Strong understanding of Care Programme Approach (CPA) and Care Management processes.
  • Demonstrable crisis assessment, risk assessment, and risk management skills.
  • Ability to work effectively both independently and within a multidisciplinary team.
  • Excellent written and verbal communication skills.
  • Experience mentoring or supervising junior staff or learners.
  • Strong caseload management and organisational abilities.
  • Ability to work rotating shifts across a 7-day service.

Preferred

  • Previous experience working within crisis or acute mental health services.
  • Knowledge of local and national mental health policies and legislation.
  • Experience with safeguarding processes, Mental Capacity Act (MCA), DOLS, and Community Treatment Orders (CTOs).
  • Familiarity with psychosocial interventions such as CBT, DBT, trauma-informed approaches, or WRAP planning.
  • Experience contributing to service development and improvement projects.
