Job Description:

We are currently recruiting for Lead Reablement workers part‑time (21 hours per week) in Cambridge. The Reablement Service provides care at home to people aged 18 and over, delivering support in an enabling way so that people can maximise their independence. The role involves delivering short‑term, person‑centered support that helps individuals regain confidence and independence in their daily lives.

Responsibilities

  • Assess individual needs and create tailored reablement plans that focus on recovery and independence.
  • Work hands‑on with service users, helping them relearn essential daily living skills such as personal care, mobility and meal preparation.
  • Monitor progress and review plans, ensuring goals are met and adapting support as needed.
  • Collaborate with health and social care professionals, families and other agencies to provide a holistic approach to care.
  • Maintain accurate records and documentation to ensure compliance and quality standards.
  • Act as the first point of contact for service users and liaise with Occupational Therapy teams and other health professionals to set personalised goals.
  • Refer users to community support services and continuously monitor their care throughout the reablement process.

Qualifications

  • Proficient written and spoken English for case recording.
  • NVQ Level 3 or equivalent apprenticeship.
  • Experience supporting people with care and support needs.
  • Knowledge of the Care Act and CQC requirements.
  • Strong communication skills, able to oversee support workers in the community setting and liaise with many health professionals.
  • Strong skills in managing and prioritising a diverse caseload, ensuring timely and effective resolution of tasks in a fast‑paced environment.
  • Excellent verbal and written communication and IT skills.
  • Awareness and understanding of equality, diversity and inclusion, and competence in safety and confidentiality.
  • Ability to travel across Cambridgeshire, access to a car and a full driving licence with business insurance.
