Support Worker – Young People with Complex Needs

Responsibilities
- Deliver person‑centred care that supports each individual’s unique needs and interests.
- Develop and maintain Support Plans, ensuring accurate and up‑to‑date information.
- Facilitate meaningful community engagement, relationships, and skill development.
- Observe and report changes in an individual’s needs to the Team Leader/Senior Support Worker.
- Safeguard the welfare and safety of all supported individuals.
- Routinely complete required paperwork, such as incident and progress reports.
Qualifications & Requirements
- Right to work in the UK.
- Satisfactory criminal records checks, ID, and address verification.
- Drivers licence preferred (funded by Glassmoon Services Ltd).
- Reference checks.
