Empowering Community Discharge Case Worker

Coventry City Council is seeking a Community Case Worker in the Community Discharge team to support timely hospital discharges for individuals with various social care issues. The ideal candidate will have experience in assessment, case management, and a passion for promoting independence.

This full-time permanent position requires strong organizational skills and the ability to work as part of a team as well as independently. Positions are available immediately.
