Are you a creative and motivated individual with the passion and drive to support people with a recent diagnosis of dementia to live well in their community? If so, you could join a National Team of Post Diagnostic Support Link Workers (PDS Link Workers) providing high‑quality, person‑centered support to people living with dementia and their families for a minimum of a year following diagnosis, in line with the Scottish Government’s minimum guarantee.

Role Overview

The PDS National Service aims to enable people to live well with dementia, centred around Alzheimer Scotland’s 5‑pillar model, so that they can move onto a period of supported self‑management. The model includes understanding the illness and coming to terms with the diagnosis, peer support, connecting and maintaining community links, and planning for future care and decision‑making to develop an outcome‑focused plan that supports their future hopes, desires, and aspirations. The role will be based in the North West of Edinburgh and involves a blended approach of virtual meetings and online groups.

Responsibilities

  • Provide high‑quality, person‑centered support to people living with dementia and their families for a minimum of a year following diagnosis.
  • Support individuals in understanding and coming to terms with their diagnosis, and in establishing peer support networks.
  • Connect and maintain links with community resources, NHS services, and other stakeholders.
  • Assist in planning for future care and decision making, and develop outcome‑focused plans aligned with the individual’s hopes and aspirations.
  • Employ digital platforms for virtual meetings and online groups as part of a blended support strategy.

Qualifications

  • Recognised relevant professional qualification.
  • Good understanding of dementia and its impact on people and families.
  • Excellent communication skills, with a warm and flexible approach.
  • Strong empathy and relationship‑building abilities.
  • Skills and knowledge at the Enhanced Level of the Promoting Excellence Framework, or evidence of working towards its completion.

Other Requirements

  • Effective team‑working skills and the ability to build and maintain relationships with colleagues, NHS professionals, and other stakeholders.
  • Strong digital skills to support virtual meetings and online support groups.
  • Full driving licence and access to a car.
