Dementia Post-Diagnosis Support Navigator

Alzheimer Scotland is looking for a dedicated Post Diagnostic Support Link Worker to provide high-quality, person-centered support for individuals living with dementia and their families. This role focuses on enabling these individuals to live well within their communities as part of a National Team.

Your responsibilities will include helping clients understand their diagnosis, connecting with community resources, and assisting in planning for future care. This position requires a recognised professional qualification and strong communication skills.
