What you will do

• Assess and manage a caseload of adults in need of social care support, ensuring the delivery of high‑quality services.
• Collaborate with healthcare professionals to create comprehensive care plans tailored to individual needs.
• Conduct risk assessments and work towards reducing potential risks for clients.
• Facilitate effective communication between hospital team members, clients, and their families.
• Maintain accurate records and documentation in line with statutory requirements.
• Provide information and support to clients and families regarding care plans and available resources.
