What you will do:

  • Deliver proactive street outreach, identifying and engaging rough sleepers, including those who have disengaged from services.
  • Undertake comprehensive assessments, risk evaluations, and personalised recovery/support plans for each service user.
  • Manage a caseload, coordinating care pathways including housing, substance misuse treatment, and mental health services.
  • Support service users into emergency, temporary, and long-term accommodation, including resettlement planning.

Person Specification:

Qualifications:

  • NVQ Level 3 (or equivalent) in Health & Social Care, Substance Misuse, Housing, or a related discipline, or relevant experience.

Registration & Compliance:

  • Enhanced DBS clearance.
  • Knowledge of safeguarding adults and children frameworks.
  • Understanding of relevant legislation including the Housing Act, Care Act 2014, and GDPR.

Experience:

  • Experience working with vulnerable adults within homelessness, substance misuse, mental health, or criminal justice settings.
  • Proven ability to support individuals with multiple and complex needs.
  • Experience of multi-agency working across statutory and voluntary services.
  • Experience delivering person-centred, recovery-oriented support in community or outreach environments.
