Specialist Peer Support Outreach Worker

We are seeking a motivated Specialist Outreach Worker, with lived experience of inclusion health, to join the Find&Treat service, supporting people living with HIV and other infectious diseases to engage with care.
This role will focus on working with individuals who are out of care or at risk of disengagement, particularly those experiencing homelessness, substance use, migration‑related vulnerability, or contact with the criminal justice system.
As a Specialist Outreach Worker, you will use your lived or relatable experience to build trust, support engagement, and help individuals overcome barriers to accessing healthcare and wider support.
Main duties of the job
- Support people living with HIV to re‑engage with care and attend clinical appointments
- Build trusting relationships with individuals who may be disengaged from services
- Undertake outreach in community settings, including hostels, day centres and other environments
- Provide practical support to address barriers to care (e.g., navigating services, attending appointments)
- Support BBV testing, TB and STI screening alongside clinical staff
- Work closely with nurses and other professionals to support continuity of care
- Identify wider health and social needs (e.g., housing, benefits, substance use, mental health) and support individuals to access appropriate services
- Maintain accurate records and contribute to service monitoring and evaluation
- Contribute to a trauma‑informed, person‑centred approach to care
This is a generalist inclusion health outreach role with a strong HIV focus. You will work flexibly across London, supporting individuals in a range of community settings.
You will be part of a supportive multidisciplinary team and will receive training and supervision to develop your skills in outreach, engagement, and health support.
About Find&Treat
About Find&Treat Find&Treat is a specialist NHS outreach service working with over 200 NHS and third‑sector partners to tackle TB, HIV and blood‑borne viruses among people experiencing homelessness and social exclusion. We take healthcare into the community, identifying infection early, supporting engagement with care, and addressing the wider health and social needs that underpin long‑term outcomes.
Person Specification
Knowledge and Qualifications
- Lived experience relevant to Inclusion Health populations, with ability to use this appropriately within a professional peer support role (including experience of homelessness, substance use, migration‑related exclusion, involvement with the criminal justice system, or other barriers to healthcare access)
- Understanding of barriers faced by people experiencing social exclusion when accessing healthcare
- Awareness of HIV, blood‑borne viruses, harm reduction, and Inclusion Health approaches
- Understanding of confidentiality, safeguarding, professional boundaries, and escalation
- Willingness to undertake training relevant to the role
- Training in peer support, trauma‑informed practice, safeguarding, or motivational approaches
- Understanding of NHS and community support services
Experience
- Experience of engaging with people experiencing social exclusion, homelessness, substance use, migration‑related vulnerability, or barriers to healthcare access
- Experience of supporting people to access appointments, services, or practical support
- Experience of working or volunteering as part of a team
- Experience of outreach, peer support, homelessness, drug and alcohol, HIV, BBV, or community health services
- Experience of supporting people with complex needs to engage or re‑engage with care
Skills and Abilities
- Ability to build trusting and professional relationships with people experiencing social exclusion
- Ability to work within professional boundaries and to elevate concerns appropriately
- Ability to work independently and as part of a multidisciplinary team
- Good organisational and time‑management skills

Communication
- Good verbal and written communication skills
- Ability to communicate effectively with people experiencing social exclusion and varying levels of health literacy
- Ability to communicate appropriately with multidisciplinary teams and partner organisations
Specific Requirements
- Ability to work flexibly across multiple London outreach locations
- Willingness to work in non‑traditional clinical environments
- Ability to undertake community‑based and home visit work where appropriate
- Commitment to safe, inclusive, and professional practice
- Full UK driving licence
