Qualified social workers are invited to join our Integrated Services for adults with disabilities at the Millennium Centre in Leasowe. As a person‑centred practitioner you will support adults with complex needs and their carers to achieve their desired outcomes.
Key Responsibilities
- Carry out Care Act assessments and complete mental capacity assessments.
- Develop personalised support plans following asset and strength‑based approaches.
- Lead safeguarding enquiries and manage related risk and reporting.
- Work collaboratively with the wider multidisciplinary team to ensure coordinated care for service users.
Qualifications and Experience
- Registered social worker with Social Work England (SWE).
- Experience in Care Act assessment, support planning and safeguarding.
- Good understanding of asset‑based and strength‑based assessment approaches.
- Welcoming applications from newly qualified staff or final‑year social work students.
