Transition Social Worker – Learning Disability

The Royal Borough of Windsor & Maidenhead is offering an exciting opportunity for an experienced Social Worker to join their Community Team for People with Learning Disabilities. This full-time role includes hybrid working and a competitive salary range of £40,192 to £44,012 per annum, along with a retention bonus of £3,000.

The successful candidate will work with young people transitioning to adult services, conducting assessments and care planning while collaborating with various health and social care organizations.
