About the role
We are looking to recruit at Level 3 Social Worker for the Mental Health Social Care Hub that works across Kirklees providing assessment and support for people with diagnosed mental health needs who meet the eligibility criteria for social care needs as defined under the Care Act 2014.
We are a small and friendly team with a number of years of experience both within mental health and adult social care.
Responsibilities
- Complete Care Act assessments, reviews, and support plans.
- Undertake strengths‑based, outcome‑focused work.
- Complete Mental Capacity Assessments and support best‑interest decision‑making.
- Contribute to safeguarding and risk management.
- Hold a managed caseload, adjusted for complexity and experience.
- Work with individuals, carers, families, and wider networks.
- Record clearly and defensibly on adult social care systems.
- Apply the Care Act, Mental Capacity Act, DoLS, and related guidance.
- Contribute to Court of Protection processes where required.
- Take part in multi‑disciplinary working.
Qualifications & requirements
- Registered with Social Work England with a recognised social work qualification.
- Evidenced working with people with mental ill health and a good understanding and practical application of key legislation including the Care Act, Mental Capacity Act, Human Rights Act and Mental Health Act.
- Sound understanding of adult social care processes.
- The ability to prioritise and meet deadlines.
- Evidence of effective partnership working with other agencies.
- Possession of a full and valid driving licence (exceptions can be made for disabled applicants). Travel is an essential part of this job and it is expected that you will use your own car for which you will be reimbursed mileage.
