Salary £36,363 – £40,777 per annum. Permanent. Short Term Support, Adult Social Care. East Lancashire.

Overview

We are seeking experienced permanent Full and Part Time Social Workers to deliver Short-Term Support across Lancashire. You will provide community intermediate care services, including reablement, residential rehabilitation, hospital discharge support, and holistic, face‑to‑face Care Act Assessments using a strengths‑based approach. The service operates 7 days a week, and team members will work one weekend in every four.

Key Responsibilities

  • Deliver holistic, face‑to‑face Care Act Assessments, reviews and support planning.
  • Provide community intermediate care services such as reablement and residential rehab.
  • Support hospital discharges and work with the multi‑disciplinary team to prevent admissions.
  • Utilise the Discharge to Assess model to conduct assessments outside hospital environments.
  • Maintain person‑centred, strengths‑based practice to support independence.

Qualifications

  • Registered with Social Work England (SWE) or applying to register.
  • Required to apply to the Disclosure and Barring Service (DBS) for a disclosure.
  • Ability to provide a car for use in connection with duties and insurance for business use.
