Package Description: Are you ready to make a meaningful difference in the lives of young individuals and deliver positive, person‑centred outcomes for people with complex support needs, including learning disabilities, autism, mental health and other complex needs. You will need to be enthusiastic, have great people skills, and a proactive approach to supporting individuals with these needs.
Location: Chard, Somerset. This unique service offers supported living, enabling people to live as independently as possible within their own homes and local community.

Job Responsibilities
- Encourage people to play active roles in their communities and promote independence.
- Support activities of daily living, including social activities, personal care, routines, meal planning and budgeting.
- Communicate effectively using the person’s chosen methods.
- Engage and motivate supporters to enhance quality of life.
- Specialise in positive behaviour support approaches when required.
- Record accurate records and report changes in health or circumstances to management.
- Work effectively as part of a team and also on own initiative.
Qualifications
- A minimum of 12 months right to work.
- Enthusiastic with excellent people skills.
- Ability to work proactively with individuals with learning disabilities, autism, mental health and complex needs.
Important information
This role requires a Disclosure and Barring Service check. The post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975. Clearance will be required.
