Empowering Care Leavers Support Worker (Hybrid)

Stockport Clinical Commissioning Group is seeking an Intensive Support Worker for the Leaving Care Team to provide tailored support to young people aged 16–25 who are transitioning to adulthood. You will support individual needs and contribute to group activities, ensuring their voices are central in shaping the services they receive.

This role requires compassion, experience with complex needs, and will work within a multidisciplinary team. A hybrid working model is available, promoting a diverse and inclusive work environment.
