Join Our Team as a Support Facilitator (Home Care) – Make a Real Difference Every Day!

Staffordshire County Council’s Provider Services team is looking for dedicated Support Facilitators to deliver person‑centred reablement care in the community. You’ll be part of an innovative and empowering service that supports individuals to thrive in their own homes.

Part‑time position with shifts from 7 AM to 10 PM, including weekends. Flexibility required, including early shifts at 7 AM and working other days to complete training.

Travel within the community (primarily Leek Moorlands) is essential; a full UK driving licence and access to your own vehicle is required. Applicants must live within 30 minutes of the base location ST13 6DW.

Key Responsibilities

  • Provide person‑centred reablement care in people’s homes, respecting dignity and promoting independence.
  • Support individuals with daily living tasks, personal care, and medication administration.
  • Communicate effectively with individuals, families, and colleagues to build positive relationships.
  • Respond flexibly to changing needs and embrace digital tools to improve service delivery.
  • Maintain accurate records and comply with health, safety, and safeguarding standards.
  • Undertake manual handling tasks as required.
  • Drive to deliver care across the local area using your own vehicle.
  • Complete interventions using assistive technology.

Qualifications and Requirements

  • Experience in social care (NVQ2 or equivalent).
  • Full UK driving licence and access to a vehicle for work purposes.
  • Physically able to support personal care and manual handling tasks.
  • Ability to work flexibly across days, evenings, weekends, and bank holidays.
  • Living within 30 minutes of the base location.
  • Enhanced DBS check and pre‑employment health assessment.
  • Full employment history from leaving secondary school for CQC regulation compliance.
