Description

Part Time / 21 Hours Per Week / Fixed Term for 12 Months. The Reablement Service is a small but dedicated team whose main goal is to enrich the lives of those returning home from hospital or experiencing a short‑term change in needs at home, ensuring residents maintain independence and healthy living in a safe manner within the comfort of their own home.

Schedule

Working a mixture of morning 7:30 am to 4:00 pm and evening shifts 1:45 pm to 10:15 pm across a rota covering 7 days a week, including weekends, bank holidays and other public holidays.

Responsibilities

  • Plan and programme all support visits within the Reablement Service across the Leicester area.
  • Coordinate visits to keep residents independent, reduce hospital admissions and provide respect and dignity.
  • Handle shift handovers, clarifying outstanding visits and ensuring coverage.
  • Respond to phone and email requests, prioritising actions and ensuring response within two hours.
  • Enter care plans and allocate visits using the OneTouch Electronic Rostering System.
  • Maintain accurate records of visits, cancellations and other status changes.
  • Generate reports for case‑responsible staff detailing actions taken.
  • Receive and process incoming referrals to the Reablement Provider Service.

Qualifications

  • Disciplined with excellent organisational and IT skills.
  • Ability to prioritise workload and work under pressure in a busy environment.
  • Strong interpersonal skills and effective communication with a diverse range of people.
  • Previous coordination experience within a social‑care setting is desirable.
  • Will undertake an Enhanced DBS check.

  • Salary: £16,231 – £17,607 per year.
