What you will do
- Conduct engaging one‑on‑one sessions and lead dynamic group work, including SMART groups, to inspire clients in their recovery journey
- Manage an active caseload of around 40-45 clients, offering tailored support for those on opioid substitution therapy, with dependency challenges, or low‑level substance use
- Maintain diligent records through comprehensive compliance documentation, including Treatment Outcome Profiles, care plans, and risk assessments, ensuring excellence in client care
- Perform thorough assessments with a deep understanding of substances, harm reduction strategies, relapse prevention, needle exchange protocols, and safeguarding measures
- Navigate case management systems with confidence, showcasing your ability to adapt swiftly to our team processes
- Work closely with team members to provide holistic support, enhancing service delivery and creating a nurturing environment for clients and colleagues alike
