Reablement Support Worker – Permanent (Bank hours optional)
About the role
Home First provides short‑term reablement care to help people stay independent in their own homes. As a Reablement Support Worker you will be part of a caring, dedicated team making a real impact in the community.
Key responsibilities
- Build and maintain positive relationships with customers and their family carers.
- Support individuals through personalised care plans and short‑term enablement strategies, offering guidance to help them make informed choices.
- Assist with personal care needs and explore alternative ways of meeting long‑term needs.
- Promote services that enable safe, independent living at home.
- Monitor progress against care plans and maintain accurate records.
Qualifications & requirements
- Valid driving licence and access to a car.
- No prior experience required – full induction and training provided.
- Willingness to travel to patients’ homes across Suffolk, including rural locations and in varying weather conditions.
- Commitment to DBS checks or police vetting as required by the council.

Working hours
Full time or flexible part‑time (7.5–37 hours per week) on an early, late, or alternating shift rota including weekends. Flexible casual hours are also available.
Travel requirements
The role involves travelling to patients’ homes across Suffolk, with required driving in all weather conditions and at varying times including evenings and early mornings.
