Job summary

To provide clinical case co‑ordination duties for a multidisciplinary integrated team. To be an effective member of the Multidisciplinary Team. Communicate effectively with staff, patients and carers as required. To complete all administration duties associated with the role and to carry out other duties under indirect supervision of a registered professional.

Main duties of the job

  • Work within MDT Discharge to Assess/intermediate Care at Home which delivers community‑based services to patients within Stockport.
  • Maintain regular telephone contacts with patients, relatives and colleagues.
  • Keep a daily account of bed and capacity availability.
  • Maintain accurate patient records and uphold confidentiality per Caldicott guidelines.
  • Update case load and document accurately on team handover.
  • Plan own workload and meet job demands.
  • Work independently using initiative within limitations and seek help when unsure.
  • Organise and participate in audits and team rota.
  • Assist with health promotion programmes, encouraging self‑awareness and self‑help.
  • Support and educate family/carers.
  • Attend mandatory training, appraisals, and maintain competencies.
  • Comply with Trust policies for personal and patient safety, prevention of HCAI, and infection control.
  • Perform other duties directed by line manager.
  • Assist during incidental contacts with patients/service users/carers/relatives.

Essential qualifications

  • Maths and English GCSE Level 3 or above
  • Experience / patient customer service
  • IT skills including Excel, Microsoft Office, Word

  • Care Certificate
  • Use of EMIS

  • Experience in patient/customer facing role

  • Two years experience in customer service role
