Social Work Support Assistant – Adult Social Care (Permanent, Full Time)

Based in the Care & Support Team at Eddie Ferguson House, Blyth.

About the role

As a key worker for adults with care needs over 18 years of age, you will be the main point of contact for support and advice to them and their families. You will lead in ensuring their safety in day‑to‑day life and help them live as fully and independently as possible. The role will include completing needs assessments, safeguarding, reviewing continuing healthcare needs and promoting self‑directed support. You will work within a Care and Support Team that includes social workers and care managers who collaborate closely with GP practices, community health services and district nurses.

Key Responsibilities

  • Carrying out assessments and working with service users to design solutions to meet their desired outcomes.
  • Managing and planning work effectively to allow monitoring and review of outcomes for individuals.
  • Undertaking Carers’ and Risk Assessments as part of the care management process.
  • Participating in the Duty System to pick up ad‑hoc work, process information and act on it under supervision.
  • Shadowing of students (e.g., Social Workers, Occupational Therapists) and newly appointed staff from other disciplines.
  • Participating in discharge planning from hospital and care planning meetings.
  • Maintaining all written and electronic records to organisational standards.
  • Providing written reports, assessment outcomes and recommendations.
  • Advocating on behalf of service users.

Qualifications & Experience

  • Level 3 qualification in social care or equivalent.
  • Experience working with adults in a social care setting.
  • Knowledge of legislation relevant to mental health and adult social care.
  • Experience working with people experiencing mental illness, learning disability and/or physical conditions in adults and older people.
  • Valid driving licence and access to a car for work purposes.
