Are you an experienced Qualified Social Worker looking to make a real impact supporting older people living with dementia?

I’m working with a well-established Adult Social Care team in Newcastle who are looking for someone who can step straight into a busy, rewarding role supporting vulnerable adults and their families.

The Role

You will be working with older adults affected by dementia, completing person-centred assessments, developing care plans, and working closely with families and partner agencies to deliver the right support at the right time.

You’ll be part of a multi-disciplinary team focused on maintaining dignity, independence and quality of life for service users.

✅ Essential Requirements (Non-Negotiable)

Qualified Social Worker (Social Work England registered)Proven experience working as a Social Worker (Adults)Strong background in assessment, care planning and safeguardingExperience working with older people and/or dementia servicesThis is not a role for someone stepping up for the first time – you must be confident managing a caseload and making informed decisions.

Key Responsibilities

Carry a caseload of adults with complex needs, including dementiaUndertake strengths-based and person-centred assessmentsDevelop and review care plans in line with the Care ActWork within Mental Capacity Act frameworks and best interest decisionsLiaise with health professionals, families and partner agenciesContribute to safeguarding enquiries and risk management
