Senior Sister – Children’s Palliative and End of Life Care

We are offering an exciting opportunity for an experienced, innovative Paediatric Nurse to lead the Palliative and End of Life (EoL) Diana Nursing Pathway within Newham Children’s Community Nursing Service. This places you at the heart of compassionate, community-based care, supporting children with life-limiting conditions and their families at the most critical moments of their journey.

The Palliative and EoL pathway lead works along the Core Nursing Pathway Lead and Complex Care Pathway Lead and sits withina widermulti-disciplinary serviceincluding Nursing(Specialist School Nursing,Asthma, Epilepsy, Continuing care)andCommunity paediatric dietitians. The Pathway Lead will also be expected to have an MDT approach to patient care with the psychology and play therapy services .

You will be responsible for the over site of palliative and EoL patients in the community setting, this may include holding a case load of your own as well as supporting and supervising the band 6 children’s community nursing to deliver palliative nursing care to children and their families.

Main duties of the job

To provide expert nursing advice and support to children who have life limiting conditions or are requiring end of life care and their families across the borough of Newham.

To work as the nurse lead for the palliative and end of life nursing pathway for children as part of the Children’s Community Nursing service (CCNS) and act as an expert resource to staff working within all teams.

To lead on End of Life care planning and deliver care for children who are End of Life.

Work in partnership with the Symptom Control team at GOSH (or other as appropriate) to provide advice and support to children & young people with life limiting/life threatening illness and their parents, carers and other professionals working with them in the community setting.

Take responsibility for line management of junior members of the team who provide nursing care to children on the palliative and end of life pathway.

To work with the Complex Health Needs Nursing Pathway lead and the Core Nursing Pathway lead within the CCNS to ensure appropriate nursing care and interventions are provide to children and their families.

Person Specification

Education, Qualification and Training

  • RSCN/ RN Child Branch
  • Evidence of continued professional development
  • Community nursing qualification
  • Qualification in palliative and end of life care

Experience

  • Significant experience at Band 6 working with palliative children & young people.
  • Significant experience of working with children complex health care needs
  • Experience of supporting children and families through the child protection process
  • Experience of leading interagency working
  • Experience of working in the community setting
  • Experience of working as a band 7
  • Experience of pathway / service leadership

Knowledge and Skills

  • Excellent knowledge and skills in paediatric nursing, including enteral, respiratory, oncology, invasive and non invasive ventilation, tracheostomy and Central Venous Access Devices (CVADs)
  • Knowledge and understanding of the child health needs of a racially and culturally diverse population
  • Ability to work flexibly, prioritise and respond to changing demands in workload.
  • Excellent organisational and time management skills and ability to facilitate these skills in others
  • Ability to support the implementation of successful change management
  • Ability to deal with complaints sensitively, avoiding escalation where possible.
  • Excellent clinical knowledge of palliative and end of life care for children and young people, including dying at home

Other

  • Excellent teaching skills with an ability to adapt these for use with health care support workers, parents, children and young people.
  • Ability to work as an autonomous practitioner and to make sound clinical judgements and decisions regarding patient care and to be responsible and accountable for choices made.
  • Excellent verbal / written communication and interpersonal skills
