What you will do

  • Conduct assessments of individuals to determine their care and support needs, implementing care plans tailored to their requirements.
  • Collaborate with other professionals and agencies to ensure that service users receive holistic and coordinated care.
  • Maintain accurate records and documentation of all interactions and care plans as per standard operating procedures.
  • Advocate on behalf of service users to ensure they have access to appropriate resources and support.
  • Continuously evaluate service delivery and adapt plans as necessary to improve outcomes for individuals.

Requirements

  • Degree or equivalent qualification in social work.
  • Current registration with Social Work England.
  • Proven experience in Social Services within the UK, working as a qualified Social Worker with substantial frontline exposure.
  • You will undertake strengths-based assessments, support adults and carers, manage safeguarding and risk, work within the Care Act and Mental Capacity framework, and collaborate with families and partner agencies to achieve positive outcomes.
  • You must be confident in managing competing priorities, making sound professional decisions, and maintaining accurate case recording.
  • Experience of Liquidlogic Adults is essential for candidates to be able to hit the ground running in this role.
  • The post requires excellent communication skills, strong adult front door experience, and the ability to work effectively within a multi-agency environment.
