About the Role

We are seeking an experienced and compassionate Adult Social Worker to join our Adult Services team. This role involves supporting vulnerable adults through assessment, safeguarding, care planning, and multi‑agency working across community and hospital settings.

You will manage a varied caseload, working with adults with complex needs including older people, individuals with physical disabilities, learning disabilities, mental health needs, and adults at risk.

Key Responsibilities

  • Manage a caseload of adult service users with varying levels of complexity.
  • Undertake assessments of need, risk, mental capacity, and safeguarding concerns.
  • Develop, implement, and review person‑centred care and support plans.
  • Carry out statutory duties in line with relevant legislation, policies, and procedures.
  • Support adults in crisis situations and respond appropriately to urgent referrals.
  • Work effectively with individuals who may be reluctant to engage with services.
  • Complete carers’ assessments and develop appropriate support plans.
  • Produce high‑quality case records, assessments, reports, and professional recommendations.
  • Work collaboratively with health professionals, housing teams, police, voluntary agencies, and wider multidisciplinary teams.
  • Promote independence, wellbeing, equality, diversity, and safeguarding best practice.

Requirements

Essential

  • Degree in Social Work (BA, MA or equivalent).
  • Registration with Social Work England.
  • Adult Social Care experience within a multidisciplinary setting.
  • Knowledge of relevant legislation including the Care Act, Mental Capacity Act, safeguarding principles, and adult social care practice.
  • Strong assessment, risk management, communication, and case management skills.
  • Ability to manage competing priorities and work independently.
  • Full UK driving licence and access to a vehicle.

Desirable

  • Experience working with Learning Disabilities.
  • Previous experience working alongside healthcare professionals.
  • Supervisory or mentoring experience.
