Adults Social Worker – Care & Support (2 positions)
Location: Aberdare – Trecynon. Working hours: PAYE £30.50 per hour, Umbrella £38.43 per hour. Hybrid working available (predominantly home‑working). Attendance required on site one day per week: Taff East Team – Tuesday, Cynon South Team – Thursday.
Role Overview
The role involves supporting adults aged 18+ with a range of needs including physical disabilities, chronic illness, learning disabilities, and mental health conditions. You will manage a varied caseload, undertake complex assessments and interventions, and work closely with families, partner agencies, health professionals, housing services, and the police.
Responsibilities
- Conduct assessments and develop care plans for adults.
- Provide interventions and support to improve independence and wellbeing.
- Collaborate with multi‑agency partners and maintain safeguarding focus.
- Maintain accurate record keeping and report writing.
Qualifications and Experience
- Social Work qualification and registration with Social Care Wales.
- Experience within Adults Services.
- Strong assessment, communication and report writing skills.
- Knowledge of safeguarding procedures and the Mental Capacity Act 2005.
- Proactive, organised, compassionate approach to practice; commitment to strengths‑based and person‑centred working.
- Experience or understanding of Continuing Health Care and Care & Treatment Planning under the Mental Health Measure (Wales) 2010 is advantageous.
