What you will do

• Hold a caseload of care leavers aged 16–25 and provide advice, guidance and practical support in accordance with statutory requirements.
• Develop, implement, and review individual Pathway Plans, ensuring they reflect each young person’s unique needs, aspirations and progress.
• Support young people in securing and sustaining suitable accommodation, education, training and employment opportunities.
• Undertake regular visits and maintain contact with young people, including those who may be disengaged from services.

Person Specification

• Relevant qualification in Social Work, Youth Work, or similar field, such as Level 3/4 or equivalent.
• Evidence of continued professional development.
• Comprehensive knowledge of key legislation, including the Children Act 1989 & 2004, Children (Leaving Care) Act 2000, and Children and Social Work Act 2017.
• Understanding of safeguarding responsibilities and corporate parenting principles.
