We are looking to recruit a Band 6 CPN to our service within the enhanced community dementia team. We are currently in an exciting period of growth and transformation, aiming to provide locality‑based specialist dementia care for pre‑, peri‑ and post‑diagnostic dementia care and support.
Role Overview
This position offers a challenging and rewarding role within the multi‑professional dementia team, contributing to shaping the developing service. The core priorities include facilitating rapid access to diagnosis, treatment, crisis prevention and intervention, preventing avoidable hospital admissions and assisting with timely discharge from hospital.
Responsibilities
- Provide well‑developed clinical assessment and lead in care planning with service users and carers.
- Manage urgent diagnostic and treatment pathways, ensuring rapid access to specialist care.
- Collaborate with a multidisciplinary team of mental health nurses, occupational therapists, social workers and support workers.
- Engage with community and primary care providers, as well as social care agencies.
Qualifications
- Band 6 level clinical experience in a dementia or related mental health setting.
- Strong clinical assessment skills and leadership abilities.
- Experience compiling care plans and working in partnership with patients and carers.
