Senior SEND Transition Worker
Location: Uxbridge / Hayes. Full time and permanent. Salary: £34,878 – £37,035.
In this challenging and rewarding role based within our Learning Support team, you will work directly with young people (Year 9 upwards) with Special Educational Needs and Disabilities, and their parents/carers, to enable them to make an effective transition to the College.
Reporting to the Head of Learning Support, you will also take a lead on all administrative duties for receiving and processing of learners with an Education, Health and Care Plan (EHCP), including leading the EHCP Consultation process and Annual Review meetings.
Key Responsibilities
- Work closely with the Head of Learning Support to lead EHC Consultation responses, ensuring compliance with relevant legislation.
- Agree initial cost estimates prior to enrolment with the Head of Learning Support, as requested by Local Authorities.
- Maintain an up-to-date spreadsheet of all correspondence regarding EHC plans and provide reminders regarding deadlines for actions.
- Foster relationships with all feeder Boroughs, including attendance at relevant local authority meetings.
- Assist the Head of Learning Support in ensuring appropriate progression for SEN learners and negotiate funding with Local Authorities.
- Assist in correspondence with local authorities, schools and parents regarding EHC plans.
- Undertake administration for learners with SEN, including the formal review process for learners with EHCPs.
- Assist the Head of Learning Support in creating and maintaining a register of SEN learners.
- Monitor completion of individual action plans for learners, ensuring appropriate records are kept and input into the student profile on the High Needs Tracking Spreadsheet.
- Plan and coordinate with SEND Transition Workers the transition and Summer workshops from discrete and mainstream provision feeder schools, liaising with Heads of School.
- Liaise with internal and external auditors as and when required.
Qualifications and Experience
- Good standard of general education (GCSE level or equivalent)
- Experience in the education sector with children or young people with Special Educational Needs and Disabilities
- Understanding of the 2014 SEND Code of Practice
- Awareness of recent government initiatives relating to SEN
- Experience with EHCPs
- Knowledge of equality, diversity and safeguarding in education
- Experience in partnership with a variety of agencies such as schools and local authorities
- Knowledge of high needs funding
- Good IT skills, including the use of Microsoft Office and databases
- Ability to maintain accurate records, both manual and on a database
