Team Manager – Integrated Hospital Discharge Service

Hackney Council is currently recruiting for an experienced Adults Team Manager to join the Integrated Hospital Discharge Service.
This is a key leadership role within a fast‑paced, multi‑disciplinary environment, focused on ensuring safe, timely, and effective hospital discharges for Hackney residents.
About the Role
As Team Manager, you will provide strong operational leadership across the service, managing a team responsible for complex discharge planning. You will play a critical role in reducing Delayed Transfers of Care (DTOC) while ensuring residents and their families receive high-quality, person‑centred support throughout their discharge journey.
You will work collaboratively with health and social care partners, driving effective multidisciplinary working to achieve positive outcomes.
Key Responsibilities
- Lead, manage, and support a team of social workers and practitioners
- Oversee complex discharge planning and decision‑making
- Drive performance to reduce delayed transfers of care (DTOC)
- Ensure timely completion of statutory assessments and interventions
- Promote high‑quality, outcome‑focused practice across the service
- Work closely with multidisciplinary teams to coordinate effective discharge pathways
- Support residents and families throughout the hospital discharge process
About You
We are looking for a confident and experienced leader who:
- Has extensive experience in hospital discharge and complex case management
- Demonstrates strong operational leadership and team management skills
- Has a sound understanding of statutory frameworks, including:
- Mental Capacity Act
- Care Act
- Adult Safeguarding
- Possesses excellent communication skills and the ability to engage effectively with multidisciplinary teams
- Thrives in a fast‑paced environment and can prioritise competing demands
- Is committed to delivering high‑quality, person‑centred outcomes
