Job Description:

Cromer Group Practice are seeking a motivated and experienced primary care Clinical Pharmacist to join our friendly and supportive practice on a 12-month fixed term contract. This role is offered on a part-time basis for 30 hours per week with remote working available.

This is an exciting opportunity to work within a forward-thinking primary care team, with the flexibility of remote working supported.

You will play a key role in optimising patient care, improving prescribing safety, and supporting the wider multidisciplinary team. The successful candidate will be supported by two experienced Pharmacy Technicians, ensuring a well-structured and collaborative approach to medicines management.

Main duties of the job

  • Deliver high-quality clinical pharmacy services to patients
  • Undertake structured medication reviews and medicines reconciliation
  • Act as an independent prescriber to manage and optimise patient treatment plans
  • Support safe, cost-effective prescribing in line with local and national guidance
  • Manage long-term conditions and contribute to chronic disease management
  • Respond to medication queries from patients and clinical staff
  • Work collaboratively with GPs, nurses, and the wider practice team
  • Support quality improvement and prescribing audit initiatives

Qualifications

  • Registered Pharmacist with the GPhC

Experience

  • Experience working in primary care (or comparable clinical setting)
  • Strong clinical knowledge across long-term conditions
  • Excellent communication and organisational skills
  • Ability to work both independently and as part of a multidisciplinary team
