Community Mental Health Triage Nurse

An exciting opportunity has arisen within the South Kensington Community Health Hub for a triage and assessment mental health practitioner.

The role is to screen referrals, undertake specialist assessments and provide brief follow‑up or facilitate an alternative service provision for people referred to the team. It is a front‑line post involving face‑to‑face work with service users and their carers. Post holders must be able to make clear and autonomous decisions based on the complexity of presentations due to the person’s health and social care needs.

Main Duties of the Job

  • Provide comprehensive, culturally appropriate, evidence‑based assessments for adults with primary mental health need.
  • Establish eligibility for health and social care services for adults with mental health needs in line with national and local policy, the Care Act 2014, Section 117 and any other relevant framework.
  • Undertake continuous risk assessment and risk management in partnership with the service‑user, their relatives and other professionals. Record and document using correct tools and systems.
  • Co‑ordinate a multidisciplinary response to crisis situations.
  • Provide direct therapeutic care to users in their own home or other community settings, using evidence‑based nursing skills and psychosocial interventions.
  • Co‑ordinate and manage all aspects of care and support, including overseeing work carried out by statutory and non‑statutory agencies. Offer supervision, training and guidance to external agencies as required.
  • Assess, plan, implement and evaluate the care provided to users, in collaboration with users, carers, GP’s and other members of the multidisciplinary team.
  • Identify carers and support them with referrals for statutory Carers Assessments and Care Plans.
  • Screen for vocational needs and refer on for full vocational needs assessment where appropriate.
  • Work in partnership with other agencies (e.g. Housing, Benefits, Social Services, etc).

Additional Responsibilities

The successful applicant will lead a caseload of patients and work closely with the Operational Team Manager, Clinical Leads/Senior Practitioners, the Inpatient Services and Home Treatment Team.

As a case manager, you will assess health and social care needs and negotiate care plans with service users and their carers, remaining objective when working with diagnostic uncertainty.

You will proactively undertake and ensure rigorous risk assessment and risk‑management strategies are applied, helping to identify and mitigate risks and encouraging individual growth and development for your patients through informed positive risk taking.

You will apply specialist clinical knowledge and skills to carry out evidence‑based interventions, which may be individual or in groups, underpinned by relevant theoretical approaches and in line with the Trust’s Research and Development Strategy; you will also lead and supervise junior staff in this practice as required.

Person Specification

Education and Qualifications

  • Current NMC Registration.
  • Evidence of post‑registration training and CPD.
  • Training in psychosocial interventions or Thorn Family Therapy/Work training.
  • Other relevant training, e.g. substance misuse.

Experience

  • Significant years post‑registration experience.
  • Experience of working in a multidisciplinary mental health team with clients with acute/severe and enduring mental health problems.
  • Work with clients with acute/severe and enduring mental health problems who are homeless or rootless.
  • Work with people experiencing mental health crisis.
  • Supervision of students and junior staff.
  • Audit and research experience.
  • Teaching experience.
  • Experience working with people who use substances.

Skills and Knowledge

  • Excellent verbal and written communication skills, including report writing and presentations; IT skills.
  • Ability to assess, plan, implement and evaluate a comprehensive care package for the identified client group, using a wide range of resources.
  • Ability to develop a comprehensive programme of intervention and support for the identified client group and their families.
  • Knowledge and understanding of the safeguarding process.
  • Leadership skills: ability to motivate others, use initiative and manage change.
  • Basic counselling and group work skills.
  • Understanding of dual diagnosis / substance misuse.
