We have an exciting opportunity for clinical pharmacists to join our pharmacy team, working across community pharmacy and frailty services. This development role would suit either newly qualified pharmacists or more experienced pharmacists who meet the higher-level criteria in the person specification. Main duties of the job

Successful candidates will split their working week between acting as the responsible pharmacist in our community pharmacies and delivering frailty services within CHCP.CHCP frailty services are delivered by a multidisciplinary team of clinical pharmacists, doctors, specialist nurses, pharmacy technicians and therapists, providing comprehensive geriatric assessment to patients in: o the Jean Bishop Integrated Care Centreo care homes,o our community bedded unitso our hospital at home frailty virtual ward (including home visits) CHCP has four established community pharmacies within its portfolio; Brocklehurst Chemists and City Health Pharmacy. We are seeking a motivated pharmacist with strong clinical skills who can communicate effectively with customers, patients, GPs and other healthcare professionals, and help our teams deliver the highest standard of pharmacy care.Our community pharmacies provide a range of NHS services, including Pharmacy First, vaccinations, NMS and DMS, and you will play an active role in delivering them.We offer training through an individualised induction programme for this unique role, alongside professional support from our wider team of established clinical pharmacists and pharmacy staff.You must have your own transport to travel across the areas where our services operate and be able to work flexibly to meet organisational needs.

Person Specification

Qualifications

  • Masters degree Pharmacy (MPharm) or equivalent pre 2000 qualification) plus one year’s pre-registration training
  • GPhC Registered Pharmacist
  • Evidence of Continuing Professional Development
  • Post-graduate training in clinical or pharmacy practice to diploma level or working towards a diploma level qualification or able to demonstrate equivalent experience

Knowledge

  • Able to apply clinical knowledge appropriately in a variety of practice settings or patient groups
  • Understanding the current national policies and legislation relating to medicines
  • Good analytical and decision-making skills around medicines information
  • Thorough working knowledge and understanding of primary care and medicines management issues
  • Familiarity with SystmOne and other GP computer systems

Experience

  • Experience of influencing others through the use of evidence-based medicine
  • Experience of working on projects where post holder uses own initiative and manages own timescales
  • Clinical experience of conducting medication reviews
  • Ability to successfully implement change (e.g. working in a multi-disciplinary team)
  • Experience in leadership, line managing staff and conducting appraisals
  • Experience of conducting evidence-based review of literature and/or journal articles
