Senior Support Worker

We have exciting new opportunities in the LYPFT Learning Disability Service for some to join our Intensive Support Team (IST). We are looking for a Senior Support Worker who has experience of working with people with complex behaviours that challenge services, and who want to work a part of a multi‑disciplinary team to support people living in the community, and avoiding the need for hospital admissions.
The IST provides assessment, support and treatment to adults who have a learning disability and complex needs including severe challenging behaviour, autism and mental health needs. The team supports people who are reaching crisis and may otherwise require an admission to hospital. The team work closely with the individual, their families and care staff to promote the person’s health and enhance their quality of life.
Responsibilities
- Provide direct support and care to adults with learning disabilities and highly complex needs in their own homes, and other related community settings.
- Implement agreed Positive Behaviour Support plans which reduce the impact of behaviours that are challenging to services.
- Demonstrate and role model constructive person‑centred approaches to the person’s family or care team to increase their ability to provide effective interventions.
- Contribute to the assessment, planning and evaluation of care provided.
- Work closely with professionals involved and the Care co‑ordinator to ensure care teams and/or family carers are able to implement the care and treatment recommended by the CLDT to prevent admission to hospital and enhance their quality of life.
Other Job Responsibilities
- The band will offer support and supervision to colleagues and provide specialist advice relevant to their clinical practice and expertise.
- They will be responsible for delivering training to team members as well as to teams outside of LD services and the wider Trust.
- The postholder will be based at St Marys Hospital and will operationally report to the Clinical Team Manager.
- The service currently operates 7 days a week. The successful candidate will work with flexibility where required to meet service and service user need.

Person Specification
Qualifications
- Foundation degree in a relevant subject, or NVQ3 level or equivalent plus knowledge of specific patient conditions, forms of therapy, acquired through training and experience.
Experience
- Has experience of working within a healthcare community setting.
- Understanding of learning disability and health related needs.
Skills
- Evidence of a range of knowledge and skill in assessment.
- Interest in working with people with a Learning Disability.
