Job summary
RDaSH are proud to be piloting this Mental Health Homelessness Team for 18 months initially. The support worker role is instrumental in the overall working practice of the team, with trauma informed, non-judgemental interventions and relational approaches being offered to provide holistic care to individuals experiencing homelessness and mental health difficulties. With a multi-disciplinary team approach, collaboratively working with partner agencies and internal services in the Trust, consistent approved interventions will be offered to all individuals currently unable to access mainstream services due to the complexity of their difficulties. The main focus of the work being to transition the individual through their journey with services, promoting autonomy and engagement, to allow for discharge from this service to mainstream agencies for on-going support.
Main duties
To implement, maintain and contribute to the delivery of high-quality care as planned and directed by health care professionals with the Mental Health Homelessness Team in Doncaster. All care and activity will be undertaken in accordance with the Trust’s vision and values.
To support individuals with identified mental health, substance misuse, physical and social difficulties for individuals as per care plans, whilst ensuring robust professional relationships are built and maintained, allowing for holistic care packages to be delivered.

Essential
- NVQ II or equivalent
- Numerate and literate
- Willingness to undertake relevant training

- NVQ III or equivalent
- Drug & Alcohol experience

- Appropriate demonstrable experience of working or caring for people with mental health problems
- Understanding of the assessment and management of risk within the client group
