Community Mental Health Nurse/ Practitioner

An exciting opportunity has arisen for a Community Mental Health Nurse / Practitioner to join our newly developing Enhanced Support Team within Community CAMHS. This role has been created to improve access for children and young people who experience barriers to traditional clinic-based care. The team works flexibly and creatively across community settings, supporting young people within their wider systems.
You will be part of a close-knit, multi-disciplinary team where responsibility is shared and held collectively. Many young people present with high levels of vulnerability, complexity, and risk (to self, others, and from others); however, this is held within a coordinated, supportive team approach rather than in isolation.
The role emphasises multi-agency collaboration, working with families, education, social care, and partner agencies to develop shared understandings and psychologically informed care plans that promote relational care.
As the team is still developing, there is genuine opportunity to shape ways of working, team culture, and approaches to engagement, risk, and intervention.
We are particularly interested in practitioners who demonstrate a strong values base, including trauma-informed, relationship-based practice; a systemic perspective; confidence working flexibly beyond traditional models; collaborative, team-based thinking; respect for multi-agency expertise; and a belief in the potential for change, even in complex situations.
Main duties of the job
Reporting into the Clinical Nurse Specialist , some of your responsibilities will include:
- Manage and take responsibility for an individual case load as an autonomous practitioner.
- Undertake comprehensive assessment, care planning, implementation and evaluation of care and treatment packages.
- Deliver a range of evidence-based therapeutic interventions to children, young people and families.
- Work in close partnership with colleagues across Specialist CAMHS, education, social care and other agencies to provide holistic, joined-up care.
- Contribute actively to multi-disciplinary team (MDT) working, service development and team effectiveness.

Person Specification
Qualifications and Training
- RMN / RN (Mental Health), Occupational Health Therapist, Social Worker
- Post registration clinical training relevant to CAMHS / Mental Health
- Teaching and Assessing ENB 998 / Mentorship or equivalent
- Degree in relevant subject
- Additional vocational training courses
- Solution focused, CBT, Motivational interviewing
Experience
- Significant Post registration experience within a CAMHS / Mental Health setting
- Experience of mental health assessment, treatment and evaluation
- Experience of multi-agency liaison
- Experience of working in a multi-disciplinary team
- Supervision of less experienced staff
Knowledge and Skills
- Broad knowledge and application of assessments and a range of therapeutic interventions deliverable to young people and families
- Application of Health, Safety and Risk Management policies
- Knowledge of a legal frameworks and practice relevant to CAMHS
- IT literacy
- An interest in the appropriate National Service Framework for the clinical area, and government policies and guidelines
- Knowledge of the principles of clinical governance and its application
- Auditing clinical standards
- Utilisation of research in practice
