Job Responsibilities
To work in partnership with the Heart Failure Specialist Nurse team in providing specialised evidence based care in accordance with the clinical management plan by working in conjunction with the senior HFSNs. The aim is to provide care and interventions until the patient reaches stability. To work independently managing a caseload of Amber patients and prioritising clinical need by using specialist assessment skills and evaluations. To assist in the promotion of early identification and interventions to reduce episodes of decompensation. By working closely with the HFSN and responding proactively to changes in condition and symptoms of fluid retention.
Qualifications
- First degree in nursing or an equivalent portfolio of evidence
- Teaching/assessing qualification
- Non-medical prescriber
- Post basic qualification in cardiac related subject/ heart failure
- Specialist training relevant to role (or willingness to undertake);
- Cannulation/venepuncture
- ECG and interpretation
Knowledge
- Relevant learning beyond registration
- Specialist up to date expertise acquired from theory acquired through CPD
- Knowledge of local policies, practices and procedures within specialist area and a good understanding of national strategic context of the NHS and knowledge of NICE guidelines.
- Evidence of proactive involvement in the teaching and mentorship of learners.
- Understanding of community working
Experience
- Substantial post registration cardiac experience and understanding of cardiac conditions
- Experience delivering community based cardiac care
