Role Overview:
- Support adults (18+) to leave hospital safely and regain independence
- Complete Care Act assessments including Needs, Risk, Mental Capacity and Best Interest decisions
- Use strengths‑based practice to deliver flexible, outcome-focused support
- Apply Prevent, Reduce, Delay principles to minimise long‑term care dependency
- Work on‑site as part of a Multi‑Disciplinary Team (MDT) with occasional home visits and collaborate closely with nurses, therapists and other health professionals
- Manage a varied caseload, prioritising early intervention and short‑term support
- Commission services including reablement, domiciliary care, assistive technology and carer support
- Participate in MDT meetings, duty functions and shared decision‑making
- Ensure all work is compliant with legislation, policy and best practice
