Role Overview

Do you want to make a difference in the lives of adults in Hampshire? Connect2socialwork urgently seeks a Social Worker to join our Mental Health Social Work Team. This is a locum role with Pay up to £38 per hour.

Responsibilities

• Provide support and therapy to individuals and families experiencing mental health issues such as anxiety, depression, schizophrenia, or substance misuse, using a strengths-based approach.
• Conduct psychosocial, risk and care needs assessments to establish diagnoses and identify required support.
• Develop and monitor individualised care plans in collaboration with clients and multidisciplinary teams.
• Undertake crisis intervention and risk management, safeguarding vulnerable individuals using the Mental Health Act, Mental Capacity Act or Care Act.
• Advocate for clients’ rights and coordinate with community, statutory, and voluntary services to ensure access to resources and promote social inclusion.
• Provide therapy and counselling through one-to-one or group sessions using evidence-based models such as CBT or DBT.
• Maintain accurate records, case notes, progress documentation, and adhere to confidentiality and ethical standards.
• Support carers and families, offering psychoeducation, emotional support, and inclusion in recovery planning.
