Job Summary
We have an excellent opportunity for a Clinical Pharmacist to join Health Alliance Primary Care Network, a friendly and supportive PCN made up of seven GP practices, caring for a patient population of over 58,000 patients. We are looking for an enthusiastic and motivated pharmacist who is either already an Independent Prescriber or is keen to develop towards becoming one. This role supports safe, effective and high‑quality medicines optimisation across the PCN.
Key Responsibilities
- Provide structured medication reviews for patients with complex medication needs, polypharmacy, long‑term conditions, frailty or other identified clinical risks.
- Support medicines optimisation across the PCN, helping patients to understand and use their medicines safely and effectively.
- Provide medication advice to patients, carers, clinicians and wider practice teams.
- Support chronic disease management through clinical reviews, medicines adjustment, monitoring and prescribing support.
- Contribute to safer prescribing initiatives, including audits, searches, high‑risk drug monitoring, prescribing safety alerts and quality improvement work.
- Support achievement of QOF, IIF, CRM, PCN DES and other contractual or local enhanced service requirements.
- Collaborate with GPs, nurses, pharmacy technicians, care coordinators, social prescribers and other multidisciplinary team members.
- Review hospital discharge letters, clinic letters and medication changes to support accurate medicines reconciliation.
- Help reduce medicines waste, inappropriate prescribing and unnecessary medication burden for patients.
- Support repeat prescribing processes, medicines queries and prescribing governance across PCN practices.
- Maintain accurate clinical records and ensure all work is appropriately coded and documented within the clinical system.
- Work within professional, legal and ethical boundaries, escalating clinical concerns appropriately.
- Engage in continuing professional development and, where not already qualified, work towards becoming an Independent Prescriber if agreed as part of the role development plan.
Qualifications
- Current UK professional registration (Pharmacy registration).
- Experience in medicines optimisation, patient‑facing clinical activity and multidisciplinary teamwork.
Working Conditions
Location: Belmont Health Centre, 516 Kenton Lane, Harrow, HA3 7LT, United Kingdom. Salary: £40,000 to £50,000 per year, depending on experience. Contract: Permanent. Working pattern: Full‑time or Part‑time.
