Job Summary

The post holder is a pharmacist who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. The role is supported by a senior clinical pharmacist for development, management and mentorship, and by a GP for clinical supervision.

Key responsibilities include providing clinical leadership on medicines optimisation and quality improvement, managing medicines safety and antimicrobial stewardship, working as part of a multidisciplinary team, undertaking chronic disease management and clinical medication reviews for patients with complex polypharmacy, and offering primary support for prescription and medication queries.

Responsibilities

  • Deliver clinical medication reviews for older people with frailty and produce prescribing recommendations.
  • Reconcile medicines following discharge from hospital, intermediate care and care homes, ensuring continuity of care.
  • Set up and manage systems to identify and support high‑risk patient groups (e.g. 4+ medicines or unplanned admissions).
  • Provide advice on medicines optimisation and develop personalised care plans for long‑term condition clinics (e.g. diabetes).
  • Review single medical problems where optimisation is required (e.g. COPD, asthma) and recommend improvements to GPs.
  • Answer enquiries from GPs, practice staff and patients; signpost to community pharmacy or other healthcare professionals where appropriate.
  • Develop positive working relationships with community pharmacists and other local healthcare providers.
  • Plan and organise personal workload, including audit, project work and training sessions for practice and community teams.
  • Maintain detailed records of audit and clinical work in a secure database.
  • Comply with professional registration and maintain competence through continuous learning and participation in appraisal processes.
  • Attend local, regional and national meetings relevant to the role.
  • Undertake any other duties commensurate with the post grade as agreed with the line manager.

Essential Qualifications

  • Masters degree in pharmacy (MPharm) or equivalent.
  • Minimum of 3 years post‑qualification experience.
  • In‑depth therapeutic and clinical knowledge and understanding of evidence‑based healthcare principles.
  • Awareness of the nature of general practice prescribing and rational prescribing strategies.
  • Excellent interpersonal, influencing, negotiating, written and verbal communication skills.
  • Good IT skills and ability to prioritise and problem‑solve.
  • Professional registration with the General Pharmaceutical Council.

Desirable Qualifications

  • Post‑graduate clinical diploma or equivalent training/experience, or Independent prescriber status.
  • Experience of motivational interviewing and ability to train others.
  • Ability to communicate complex and sensitive information clearly to diverse audiences.
  • Experience in planning, managing and reviewing medication optimisation issues in core long‑term condition areas.
  • Familiarity with GP systems and ability to analyse complex technical information.
  • Member of or working towards Faculty membership of the Royal Pharmaceutical Society.

Professional Registration

  • Essential: Registration with the General Pharmaceutical Council.
  • Desirable: Membership or training towards Faculty membership of the Royal Pharmaceutical Society.

Eligibility

Applicants must have current UK professional registration.

Salary and Benefits

£45,716 to £53,000 per annum, permanent contract, full‑time working pattern.
