Advanced Crisis Hub Practitioner

This role is hosted by South London and Maudsley NHS Foundation Trust and works in partnership with South West London and St George’s NHS Trust and Oxleas NHS Trust.

Main duties

  • Provide senior oversight and lead the shift, ensuring all calls are handled in a professional manner, screened and prioritised.
  • Demonstrate risk assessment and management skills, making decisions that best serve the safety and needs of service users, including transferring calls to specialist services such as CAMHS.
  • Assess high‑risk or complex patients, offering management advice and appropriate clinical interventions to support junior team members.
  • Operate within the legal requirements of the Mental Health Act and exercise independent judgement in assessment and diagnosis of mental health conditions.
  • Maintain current clinical and pharmaceutical knowledge, critically appraising and applying information in practice.
  • Liaise with carers, incorporating their views into clinical formulation and management.
  • Conduct clinical triage of calls, escalating complex calls to the service and deciding the correct pathway, including referral or signposting to downstream services.
  • Apply appropriate frameworks such as Trusted Assessment for difficult calls and ensure correct referral and escalation procedures are followed.

Person Specification

Qualifications

  • Recognised Professional Qualification in Nursing (RMN), Social Work (Dip SW/CQSW) or Occupational Therapy.
  • Post registration qualification in a relevant specialty or equivalent experience.
  • Mentorship for RMNs.
  • Evidence of continuing professional development.
  • Master’s Degree.

Experience

  • Experience and knowledge of quality, standard setting and audit.
  • Experience in managing complaints and quality assurance.
  • Experience of working within acute and crisis services.
  • Ability to review performance data and suggest ways of improving performance.
  • Experience of working within CAMHS services and ability to manage crisis and high‑risk presentations of under‑18 year olds.
  • Comprehensive local leadership experience within a specialty.

Knowledge/Skills

  • Ability to analyse complex issues, gather relevant information and exercise sound judgement in reaching appropriate conclusions.
  • Strong commitment to improving patient services with a clear focus on achieving performance goals.
  • Competence in providing clinical supervision and people‑management skills.
  • Excellent interpersonal, communication, negotiation and influencing skills, demonstrated by the ability to engage others.
  • Excellent written and verbal skills.
  • Knowledge of a full range of therapeutic skills and approaches, with further training in the specialty and management practice.
  • Knowledge of STORM and/or ADASTRA telephone and EPJ packages.

Salary

£55,524 to £62,652 a year inclusive of HCAs (outer).
