Salary range: £51,228 – £54,360 per annum

Work location: Hammersmith & Fulham Civic Campus, 1 Unity Square, Hammersmith, London W6 9JU

Hours per week: 36

Contract type: 6x Permanent

Vetting requirements: Enhanced DBS Check

About the role

We are seeking experienced, qualified Occupational Therapists to support independent living and prevention within Adult Social Care. As a HCPC‑registered practitioner with strong post‑qualifying experience, you will work across complex practice areas including functional assessment, strength‑based planning, equipment and adaptations, re‑ablement, housing‑related interventions, and the application of the Care Act. Your professional judgement will be trusted, and you will collaborate with social workers, health colleagues, and community partners to deliver outcome‑focused care.

Responsibilities

  • Conduct functional assessments and develop strength‑based plans
  • Provide equipment and adaptation advice
  • Deliver re‑ablement and housing‑related interventions
  • Apply the Care Act in practice
  • Work collaboratively with multidisciplinary teams

Qualifications

  • Occupational Therapy qualification
  • Current HCPC registration
  • Demonstrated experience in adult social care practice
