Job Summary
Band 5 Community Nurse with HCRG Care Group based at Sheppey Community Hospital. The role is part of the North Kent Community Nursing Service (Swale) covering Sheppey, Sittingbourne, and surrounding villages. The position offers full UK driving licence and vehicle access as an essential requirement.
Main Duties
Provide holistic, patient‑centred care to patients and their families while delivering high service standards and sharing best practice. Support the clinical team through regular meetings and one‑to‑one sessions with managers. Use clinical equipment to maintain quality standards and support continuous improvement within CQC domains.
Ideal Candidate
- NMC/1st level registered Nurse with strong communication, organisation, and flexible skills.
- Full UK driving licence and access to a vehicle for work purposes.
- Proficient with IT systems and Microsoft Office (Outlook, Word, Excel).
